The best place to start is the logs. In particular: /opt/kaltura/log/kaltura_api_v3.log and the relevant batch logs under /opt/kaltura/log/batch.
You may use the kaltlog and allkaltlog aliases to locate erroneous patterns. Once you have them, trace back to where the flow starts and paste the relevant log lines here so I can help you find the root cause.
